We've long rolled out the
`COURSEWARE_COURSE_NOT_STARTED_ENTERPRISE_LEARNER_ERROR` setting toggle so
it should be safe to remove.
I'm completely at a loss as to why course access checks are not
returning the `course_not_started_enterprise_learner` error code when it
should, but it does in stage/local, so this PR is grasping at straws.

Normally, the course API would return an access error_code of
`course_not_started` if the course has not started yet. This change
breaks that up into two codes:
* if the course has not started:
* return error_code=`course_not_started_enterprise_learner` if the
learner is enrolled as a subsidized enterprise learner.
* else, return error_code=`course_not_started`.
This supports a change to the frontend which will interpret
`course_not_started_enterprise_learner` differently and trigger a
redirect to the enterprise (B2B) learner dashboard instead of the B2C
dashboard.

Gate access to exam content by requiring an access token. This is a signed JWT issued by the edx-exams service that grants a user access to a sequence locator for a short lived window while an exam is in progress. This feature only applies to courses using the new exam service instead of edx-proctoring.
When a student submits a problem answer, the state is stored in a
StudentModule record containing answer, score, correctness, etc. The
record, though, is updated in multiple steps within the single request
(first the grade is updated, then the state is updated separately).
Each partial save would trigger a separate StudentModuleHistory record
to be stored resulting in duplicate and inaccurate historical records.
This solution uses the RequestCache to track within a request thread
which StudentModules are updated and a single corresponding
StudentModuleHistory id. If multiple update actions occur within the
request cycle, then modify the history record that was already
generated to ensure that each submission only results in one
StudentModuleHistory record.

As part of dissolving our sub-projects in edx-platform, we are moving this package under the xmodule directory.
We have fixed all the occurences of import of this package and also fixed all documents related references.
This might break your platform if you have any reference of `import capa` or `from capa import` in your codebase or in any Xblock.

This is a first stage for removing the LegacyWaffle* classes.
LegacyWaffleFlag usage replaced with WaffleFlag;
LegacyWaffleSwitche usage replaced with WaffleSwitch;
New CourseWaffleFlag added to the temporary module __future__ as FutureCourseWaffleFlag;
Updated all the imports to use CourseWaffleFlag from the __future__ module;
BREAKING CHANGE: A number of toggle related constants (e.g. ENABLE_ACCESSIBILITY_POLICY_PAGE)
changed types. They were strings, and are now toggle instances (e.g. WaffleSwitch). Although the entire
refactor should be self-contained in edx-platform, if any plugins or dependencies were directly
using these constants, they will break. If this is the case, try to find a better publicized way of
exposing those toggles.
