This helper is used by the LMS, CMS, _and_ `openedx.core`,
so let's move it to `openedx.core` to reduce import complexity.

Previously, unicode would result in a UnicodeEncodeError as a result of
the format string being str instead of unicode.
Additionally, the log calls would crash with a traceback (while not
affecting the program flow).
Add a new waffle switch that allows us to assume zero grades for
learners who have no entry previously recorded, and another to
disable persisting grades for unengaged learners.

Renamed django signal from GRADES_UPDATED to COURSE_GRADE_CHANGED
Hooks the pre-existing grades code into our new PersistentSubsectionGrade data
model. Includes test updates, and some minor changes to the data model that
were discovered in testing.
